The question is when FSD is safer than humans. The problem is we as humans will ignore millions of accidents avoided by FSD that always has instant reaction times and doesn't get distracted, and we'll be scared of it because of rare edge cases where a human may have been able to do better.

The difference in management and results has been amazing. Bezos set up a traditionally-managed launch company and threw billions at it for twenty years hoping it would produce, and they haven't even made it to orbit. They can't even produce the engines so Vulcan Centaur can make it to orbit.

Musk set up a more startup-like company using agile development methods, and they were running America's workhorse rocket within fifteen years (when they surpassed Atlas V).

There are many examples of a necessary UI fix though. John Denver is dead because someone put a fuel tank switch up behind the pilot seat, and reaching for it could cause disorientation. So could the radio controls on an F-111 when pulling a certain maneuver.
